All of our applications are online. Your materials, including narrative, budgets, audits, progress reports, board lists and other attachments will need to be uploaded to our web portal.

Please refer to your specific application to determine required forms.

Select the form you require from those below. Save to your computer, and once completed, upload the final document when requested to do so during the application process. All agencies are expected to use our templates. Forms from other organizations will not be accepted.

Progress Reports

    As a condition of receiving a grant, grantees agree to submit a written self-evaluation one year from the application date. It is a candid appraisal of achievements, challenges and lessons learned for the project. The report is provided to advisory committees for their review and becomes part of the grant consideration process for all future applications. If you received funding from us in the past year for the same program for which you are submitting an application, you are REQUIRED to submit a progress report with your next application.
