Instructions and Forms
To help you prepare a more effective proposal, we have created a set of instructions for you use.
The application form is online. The first time an organization applies to the Foundation through our online application system, it creates an Organization Account to use for each application going forward. Please use the same account you have previously used, or create a new one if this your first time applying online. You may contact Foundation staff to find out if your organization already has an account in our system.

The following is a list of forms you will need during the online grant process. Select the form you require. Save to your computer, and once completed, upload the final document when requested to do so during the application process. All agencies are expected to use our templates. Forms from other organizations will not be accepted.
These forms may be required for select grant programs. Be sure to review the grant application BEFORE downloading and completing these.
- Progress Report
Grantees should download and SAVE the Progress Report for further editing, printing or archiving:
Download the document, then "Save As". Please note if document is not "Saved As" onto your computer or elsewhere, changes may be lost.
If you are unable to access this document, please call 508-755-0980 to receive it by email or as a paper copy.
Progress reports are due one year from previous application grant date.* Example: March 15, 2015 is the discretionary grant deadline. The progress report would then be due on March 15, 2016.
* Rosemary Davis Memorial Fund progress reports are due upon the completion of the program or project.
* Mini Grant Fund progress reports are due upon the completion of the program or project.
