Please supply the following items:

  1. Grant Application Coversheet, MS Word
    This form may be saved for further editing, printing or archiving. Download the document, then "Save As".
  2. Program Budget Summary, MS Excel only
    NOTE: Complete columns titled "Description of Funding Source" and "Current Year FY" only.
  3. Organization Budget Summary*, MS Excel only
    *For Community Ministries applications, please provide a current and complete board-approved organization budget with comparison to actual year-to-date INSTEAD of this form.
  4. A letter on organization letterhead, 2-3 pages in length and signed by the Executive Director, describing the project, rationale, specific results and evaluation plan.
  5. Current list of board of directors with affiliation 
  6. Relevant supporting materials (optional); please limit to three items
  7. 501(c)(3) Tax Exempt Letter -- OR --
    Agreement with Fiscal Sponsor AND its 501(c)(3) letter, contact information, and financial statement
