Forms to Establish Funds and Gifts

For your convenience, we’ve included several forms that will cover most transactions you and your clients are likely to conduct with the Foundation. Of course you have wide discretion in the choice of wording you choose to employ in these forms, which simply offer suggested text.

I give, devise and bequeath to Greater Worcester Community Foundation, a Massachusetts non-profit corporation (the "Foundation"), the following assets:

[insert specific assets, dollar amount, percentage, or residuary]

This gift will create an unrestricted fund (the “Fund”) for charitable purposes in accordance with the Articles of Association of the Foundation.

The Fund shall be named The _______________________ Fund.

Annual distributions from the Fund, as determined in accordance with the distribution policies of the Foundation, shall be paid to or for the support of ____________________________ with the understanding that the Articles of Association of Greater Worcester Community Foundation gives its Board of Directors variance power should "circumstances have so changed since the execution of this instrument as to render unnecessary, undesirable, impractical or impossible a literal compliance with the terms of this instrument."

I bequeath __________________________________ (describe dollar amount, property to be given, or proportion of your residuary estate) to Greater Worcester Community Foundation, a Massachusetts non-profit to be added to the _______________________ Fund, a component fund of Greater Worcester Community Foundation, and I direct that this bequest become part of this Fund.
