REQUIRED FORMS

Please supply two complete sets of the following forms and documents for all competitive grants (see NOTE below for exceptions). Applicants may download and SAVE each of the four linked documents:

  1. Grant Application Coversheet, MS Word

    MS Word form may be saved for further editing, printing or archiving. Download the document, then "Save As".
  2. Application Instructions and Attachment Checklist, PDF
  3. Program Budget Summary, MS Excel only
  4. Organization Budget Summary*, MS Excel only
    *Not required for UniBank applications.
  5. Narrative (no more than four pages, plus one additional page for renewals)
  6. Current list of board of directors with affiliation (one page)
  7. Progress report; required for Renewal Applications
  8. Letters of understanding with collaborators; as applicable
  9. Relevant supporting materials are optional; please limit to three items
  10. 501(c)(3) Tax Exempt Letter OR Agreement with Fiscal Sponsor AND its 501(c)(3) letter AND contact information, and financial statement (audited or 990)
  11. Organizations with budgets over $250,000 should submit an independent auditor's report.

    Organizations with an annual budget less than $250,000 should submit an audited financial statement or financial review. If this documentation is not available, a board-approved statement for the most recently completed fiscal year and the organization’s most recently filed IRS Form 990 should be provided.

    For organizations using a fiscal sponsor, the fiscal sponsor's financial material is required.

STEPS TO APPLY

  1. Read the guidelines, application form and supplemental information for the particular grant program to which you are applying.. You may also want to review our annual report and grants listings, available on our website.
  1. If you have questions after reading our materials, please call the foundation to speak with a Program Officer who will be happy to provide guidance and clarification.
  2. Complete all questions in the narrative portion, fill out the cover sheet and compile the budget and other required enclosures. Make two complete sets. Do not bind or staple your proposal. Be sure to give us the name of a contact person who will be available to answer questions during the review process.
  3. Mail or hand deliver the complete proposal package so that it is in our office by the deadline. You will receive a postcard verifying that your proposal was received.
  4. Every eligible application that is received by the deadline is given full consideration.During the grant review process a Program Officer, whose job is to understand and provide evaluative information about applications, will be in touch with the contact person you have provided
  5. All applicants will be notified of a decision at the conclusion of the application review process.
  6. If your proposal is funded, you will receive an award letter. Money is released only after you return a signed copy of your award letter to us.

APPLICATION DEADLINES

Applications must be received by the 15th of the month in which the grant cycle falls.

Complete proposal packets must be received in our office by 5:00 pm on the published deadline dates (or the next business day if a weekend or public holiday). Late proposals will not be considered in the cycle.

We do not accept electronic submission of proposals, by FAX or e-mail.
