Progress Report

SELF EVALUATION
As a condition of receiving a grant, grantees agree to submit a written self-evaluation. It is a candid appraisal of results and challenges, and a financial progress report for the project. The report is provided to advisory committees for their review and becomes part of the grant consideration process for all future applications.

 

REQUIRED FORMS

Grantees should download and SAVE each of the two documents shown below:

  1. Progress Report Form (two pages) , MS Word
    MS Word form may be saved for further editing, printing or archiving. Download the document, then "Save As".

  2. Program Budget Summary (one page), MS Excel

If you are unable to access these documents, please call 508-755-0980 to receive them by email or as a paper copy.

 

PROGRESS REPORT DUE DATES

Progress reports are due 11.5 months after a grant has been awarded. Due dates are based on the application deadlines for each grant cycle. Reports must be received before any future funding may be considered.
